Importance Of Your ID Card
You will receive your permanent Identification Card from your insurance plan sponsor.
You must present this Identification Card and identify yourself as a PrimeSource Health Network member whenever you visit a PrimeSource provider. Please make sure all family members using PrimeSource providers are aware of the importance of presenting the ID Card at the time of service.
The benefits of presenting your ID Card are as follows:
- Providers’ staff are able to verify your eligibility
- Helps to ensure that you will receive the highest level of benefit under your medical coverage plan
- Eliminates the need for payment at the time of service, other than normal co-payments, if applicable
- Enables the provider to submit claim forms on your behalf
